Traditions Health Acquires Agencies in California, Nevada, and Kansas
Nashville, TN – Traditions Health, LLC (“Traditions”), a national hospice and home health provider, announced that it has acquired four hospice entities in California, one hospice entity in Nevada, and a home health and hospice provider in Kansas.
The entities located in California and Nevada were previously supported by Pursue Health, LLC, and include Sonata Hospice (based in San Diego, CA), Cadence Hospice (Orange, CA), Pacific Crest Hospice (Pasadena, CA), Sonata Hospice of Riverside County (Temecula, CA), and Sonata Hospice of Las Vegas (Las Vegas, NV). The acquisition strategically expands Traditions’ presence in Southern California and establishes Traditions’ footprint in Nevada. Stoneridge Partners served as the exclusive advisor to Pursue Health.
In Kansas, Traditions acquired Serenity Health Management, a provider of home health and hospice services in Wichita and Hutchinson. The acquisition of Serenity expands Traditions’ reach to south-central Kansas and establishes its first home health presence in the state. Agenda Health served as the exclusive advisor to Serenity.

Provider-Vendor Hybrid PathWell Acquires Caring Angels Home Health
The home health company PathWell began as a technology company aiming to help other operators with back-end issues. But when COVID-19 hit, it changed direction to become a provider itself, equipped with the capabilities it once exclusively sought to give to others.
PathWell has now grown into a multi-state provider through expansion and acquisitions. And on Wednesday, the company announced it acquired Caring Angels Home Health, a business with a significant presence in both Virginia and West Virginia.
“We have a really solid foundation, and now it’s just about pushing volume through that foundation,” Raman Brar, the CEO and co-founder of PathWell, told Home Health Care News. “It’s a scramble right now to acquire as many quality businesses that are available. But we’re still very selective in terms of what we buy, in terms of the quality of businesses, the patient census and the location.”

Bristol Hospice Acquires Two California Providers
Bristol Hospice has purchased two California providers from the same owner — A Plus Hospice and NorCal Hospice. Financial terms were undisclosed. The deal comes as companies try to complete their transactions before year’s end, due to the potential for capital gains tax hikes in 2022.
Bristol is a large multi-regional player that has grown seven times larger since Webster purchased the company in 2017. The hospice operates more than 45 locations across 13 states, and its EBITDA exceeds $70 million, according to PE Hub.
M&A Advisory Firm Agenda Health represented A-Plus and NorCal in the transaction.
“Territory and growth were probably two big factors for this opportunity. Bristol has been a leading buyer in that section of the country for quite some time,” Agenda Health CEO Al Veach told Hospice News. “That area of the country is their home turf, and so they’re always going to be competitive in that marketplace. This particular opportunity is a very healthy, attractive business and headed in the right direction, to say the least.”

St. Croix Hospice Purchases Three Companies from LifeCare Collective
St. Croix Hospice, a portfolio company of the private equity firm H.I.G. Capital, has acquired three Missouri companies from LifeCare Collective — HomeCare of Mid Missouri, Nurses & Company and Transitions Hospice. Financial terms were undisclosed.
The deal adds four locations to St. Croix’s footprint, bringing their total number of Missouri offices to six. Texas-based M&A advisory firm Agenda Health facilitated the transaction.
“This transaction was a really good fit for both. [LifeCare CEO Bobby Robertson] is very strategic. He sets his target for where he can maximize the potential of the business,” Al Veach, founder and CEO of Agenda Health, told Hospice News. “He felt like it was a good time for one of the larger entities to take it and bring it to the next level. We found a great partner that was able to meet that financial requirement and was really interested in Missouri in a big way.”

Jet Health, Inc. Acquires Signal Home Health & Hospice in San Antonio, Texas
Jet Health, Inc., a leading regional provider of home health and hospice services, announced today the acquisition of Signal Home Health & Hospice, a San Antonio, Texas-based company. The acquisition marks Jet Health’s seventh acquisition and furthers the Company’s strategy to build a premier home health and hospice provider across the western and southwestern U.S.
Founded in 2010, Signal Home Health & Hospice primarily serves patients in the greater San Antonio area as well as adjoining counties, providing hospice services, skilled nursing, physical therapy, occupational therapy, speech therapy, social workers and home health aides.
“Jet Health got its start in Texas, and we have been looking for opportunities to build density in the state by acquiring quality providers. San Antonio is the second-largest market in Texas, and the Signal Home Health & Hospice acquisition provides us the ability to further expand our presence throughout the state. This acquisition further reflects Jet Health’s attractiveness to sellers seeking buyers that can provide not just a fair purchase price, but also one that shares their culture and commitment to quality care,” explained Stacie Bratcher, Jet Health’s chief executive officer.
Signal Home Health & Hospice Owner Robert McDowell added: “I spent more than ten years building our company into a provider of quality home health and hospice services. Our strategy for success was simple – recruit and retain top-notch professionals who deliver excellent clinical care and service. It was critical that the company to whom I sold my business shared those values and would afford our team the opportunity to continue to execute this mission. As we conducted the acquisition process and spent time with Jet Health’s leadership and diligence teams, it was clear that this was in fact the right choice.”
Terms of the transaction were not disclosed. Funding for the transaction was provided by Stifel Bank, SV Health Investors and Health Enterprise Partners. Stradling Yocca Carlson and Rauth acted as legal counsel to Jet Health. The transaction was facilitated by Agenda Health, a healthcare-focused Mergers & Acquisitions advisory firm, headquartered in Austin, Texas.

Agape Care Grows in Georgia with Integrity Hospice Acquisition
South Carolina-headquartered Agape Care Group has acquired Integrity Hospice in Dublin, Ga. This boosts Agape’s footprint to cover 45% of the Peach State in addition to 46 counties in South Carolina. Financial terms of the transaction were undisclosed.
Agape, a portfolio company of the private equity firm The Vistria Group, is pursuing a three-pronged growth strategy in the Southeast focused on same-store admissions, de novo locations and acquisitions. The objective is to build density and to create a platform that performs similarly in each one of its markets, Agape CEO Troy Yarborough told Hospice News. The company has additional acquisitions in its pipeline for 2021 and 2022.
“As the health care landscape continues to change, there is an increased need for quality hospice and palliative care,” Yarborough told Hospice News. “This opportunity allows us to expand our reach and provide access to care for more patients and families across the state.”

Choice Health at Home Announces a Third Oklahoma Acquisition of Home Health and Hospice
TYLER, Texas, July 27, 2021 /PRNewswire/ — Choice Health at Home, LLC (“Choice”) formally announced the acquisition of Oklahoma-based Alpha Home Health and Hospice (“Alpha”). For Choice, the transaction marks the third in Oklahoma since January 1, 2021, creating a substantial service footprint in the state.

Pennant Group Purchases Open Heart Hospice
The Pennant Group, Inc. (NASDAQ: PNTG) has acquired Amarillo,Texas-based Open Heart Hospice. The organization will rebrand as Kinder Hearts Hospice of Amarillo. Financial terms are confidential.
The Pennant Group’s (NASDAQ: PNTG) acquisition strategy is fueling growth in both the near- and long-term for the company’s home health and hospice segment. Pennant has been very active in the M&A market during the past two years, with additional transactions waiting in its pipeline.
