Behavioral Health
At Agenda Health, your intricate transition challenges as a behavioral health provider are not lost on us. Our faith-driven M&A guidance combines decades-long expertise with a deep appreciation for your work. We connect you with partners who share your values in treating mental health and addiction with compassion while securing fair market value in selling your practice.
Upholding Your Work in Behavioral Health
As a behavioral health provider, you understand the profound impact your work has on individuals, families, and communities. You’ve built a practice that not only treats symptoms but also changes lives through empathetic mental health and addiction care. At Agenda Health, we recognize the sacred trust you’ve established with your patients and staff. We facilitate behavioral health organizations like yours through transitions that honor both your clinical excellence and your commitment to healing.
Meeting Rising ABA Demand with Expertise
The growing prevalence of autism spectrum disorder, which is now diagnosed in roughly 1 in 36 children, continues to drive demand for Applied Behavior Analysis (ABA) services across the country. As the field evolves, providers face increasingly complex decisions around staffing models, reimbursement rates, and clinical quality.
We’ve worked closely with ABA organizations of all sizes to navigate these challenges and capitalize on market momentum through strategic sales and capital raises. With active interest from both private equity and strategic buyers, many founders are finding this to be a compelling time to assess their long-term goals and consider next steps.
Navigating Growth & Opportunity in IDD Care
We’ve advised a wide range of Intellectual & Developmental Disabilities (IDD) providers through successful transactions and growth initiatives, bringing deep experience in both group home–based care and in-home support models. As states continue to shift toward community-based services and consumer-directed care, the IDD sector has become a focal point for mission-driven investors seeking scalable, high-impact platforms.
Operators must navigate complex Medicaid reimbursement structures, staffing challenges, and regulatory variation across markets, which are all areas where we bring meaningful insight. For providers with strong compliance, outcomes, and infrastructure, today’s market presents a compelling environment to explore strategic options or growth partnerships.
Facilitating Transitions for SUD Care Providers
The demand for high-quality Substance Use Disorder (SUD) treatment continues to grow, driven by rising overdose rates, expanded insurance coverage, and a broader push for integrated behavioral health services. With over 40 million Americans affected by substance use each year, the need for accessible, outcomes-driven care has never been greater.
We’ve advised SUD treatment providers across a range of models, including residential and PHP to MAT and outpatient programs, navigating complexities around licensure, census stability, and payer strategy. As interest from both financial and strategic buyers accelerates, many operators are finding this to be a strong environment to evaluate strategic options.
Meeting Advancements in Counseling & Psychiatry
The counseling and psychiatry landscape has seen sustained growth as awareness of mental health needs expands and access to care improves. Nearly 1 in 5 U.S. adults experience a mental illness each year, fueling demand for both therapy and medication management across age groups and acuity levels.
We’ve worked with behavioral health providers offering in-person, virtual, and hybrid care models, and we understand the operational and clinical dynamics, from payer mix and provider utilization to licensure and compliance. With growing investor interest in scalable, tech-enabled mental health platforms, it’s an opportune time for providers to assess their growth or exit strategies.
“Agenda gave us peace of mind and ease in selling our business. It’s such a big decision and undertaking, and Agenda takes something that’s probably one of the biggest decisions that a company owner is going to make and makes it easy and smooth. I would highly recommend it to anybody that is in our same kind of situation.” – Roy and Pam Payton, Former IDD Business Owners

Active Day
Behavioral Health
2025
Active Day announced in April 2025 its acquisition of Illinois-based PremierCare Consulting, marking a strategic expansion in the intellectual and developmental disabilities (IDD) services market. Agenda Health, an Austin, Texas-based M&A advisory firm specializing in healthcare, acted as sell-side advisor to PremierCare.
Based in Naperville, PremierCare is contracted with the Illinois Division of Developmental Disabilities to provide person-centered planning and case management. Its team of Qualified Intellectual Disability Professionals and clinicians delivers in-home and community-based support, helping individuals live more independently through tailored care.
Active Day provides home and community-based services in the United States. Founded in 1988 and headquartered in Trevose, Pennsylvania, the company serves more than 7,500 members, offering healthcare and community services to people with disabilities and older adults. Active Day has pursued a dual strategy of organic growth and targeted acquisitions to expand its reach and community impact.
Active Day’s acquisition of PremierCare Consulting caps a busy April 2025, as the company also acquired South Carolina home care providers AllCaregivers, Inc. and New Generations Home Care of Florence, Inc. earlier in the month. This marks Active Day’s fourth transaction of 2025, following its earlier acquisition of Goldencare, a South Carolina-based adult day and home care provider.
The LevinPro HC team spoke with Matt Donnelly, CEO of Active Day, and Randy Decko, Senior Director at Agenda Health, about what brought these organizations together and the broader trends shaping M&A in this space.
Active Day’s 2025 acquisitions reflect a deliberate strategy to build a national platform integrating IDD and older adult services through in-home, day program and community services. This approach positions Active Day to meet rising demand for comprehensive care models across diverse markets. According to Donnelly, PremierCare stood out because of its strong reputation for person-centered planning and its alignment with Active Day’s mission of enriching community life.
“Its commitment to quality, community engagement and a highly skilled team made it clear that a partnership would enhance our shared impact,” said Matt Donnelly. “We saw in PremierCare a cultural and philosophical alignment that complements the services we provide, and an opportunity to strengthen our regional presence with a like-minded provider.”

Abound Health is expanding its market presence in North Carolina with the acquisition of Heartspring, a provider of mental health and intellectual and developmental disabilities (IDD) services. Agenda Health, an Austin, Texas-based M&A advisory firm specializing in healthcare, acted as the exclusive sell-side advisor in the transaction. The deal strengthens and expands Abound’s footprint in the IDD market.
Heartspring is a non-profit organization dedicated to providing resources and services for individuals with developmental disabilities. It was founded in 2001 by Richard Williamson. While Abound Health delivers comprehensive services across North Carolina, including day programs, group homes and supportive employment for children and adults with IDDs. Operating for more than 20 years, the company emphasizes person-centered care and community integration to enhance client independence.
Heartspring has carved out a niche in Charlotte with its focus on tailored IDD care. And Abound Health, expanding its behavioral health presence, saw an opportunity to integrate a partner it had long respected. The LevinPro HC team spoke with Alex Veach, Director of Transaction Services at Agenda Health, to discuss how this deal came together.
Heartspring turned to Agenda Health to find a buyer that would keep its focus on tailored IDD care intact. Agenda targeted groups active in IDD, a space drawing steady interest from strategic players like Sevita, which acquired Shorehaven and BrightSpring’s community living business this year, and VIVA Pediatric Healthcare, which bought two Texas pediatric care centers in March 2025.
“Agenda facilitated a competitive market process for Heartspring that produced multiple offers,” Veach said. “Heartspring ownership decided that the offer received from Abound Health best aligned with their exit goals.”

Having spoken with several agencies before making our decision, the transparency of Agenda stood out. That made it real simple: if we were going to do business, we wanted to do business with a firm of that integrity. They were present at every meeting, and the prep and debrief conversations meant we always knew what to expect. As we got closer to the end of the deal, it was new terrain for me and I didn’t know how to navigate it, but Agenda did a great job helping us work through those difficult talking points. In the end, they came through with everything promised on the front end, and that exceeded expectations.
Initially, the process felt overwhelming since it was my first time selling a business, but Agenda Health helped ease my concerns and answered all my questions. Agenda filled in the gaps where I lacked understanding … in the end, Agenda made a very complicated process feel simple.”
The Agenda Team was patient and went at the pace that we needed to go, which was important for us. They also provided education on the various dynamics of the process, which we greatly appreciated. We were introduced to a variety of buyers, which opened our eyes to various possibilities and gave us a good idea of what we were looking for in an acquirer. We would highly recommend Agenda for the value they bring.”
My overall experience working with Agenda was very positive. The team was professional, supportive, and ultimately delivered a great outcome. The valuation they provided was very insightful and valuable in understanding my company’s position, and I really appreciated the constant communication throughout the process, which kept me informed at every stage. Overall, the guidance, insights, and consistent support were invaluable in reaching a successful outcome, and I’m pleased with the experience.”
Agenda really understands ABA – they understood the differences in my practice and others and had great knowledge of the ABA-specific M&A market. They were able to clearly express our mission and program to potential buyers and understood what made my business different. Everyone was aligned on my goals and worked hard to accomplish them.”
Working with Agenda Health was a first-class experience. As first-time sellers, their clear communication and steady support helped calm our nerves throughout the process. We hadn’t planned to sell until Agenda provided a valuation, but we never felt pressured in this process. After Agenda presented our valuation information and the pros and cons of selling, we felt equipped to make an informed decision and committed to partnering with Agenda to sell our business.”
Agenda Health’s deep understanding of the ABA space and knowledge of the market made all the difference in my transaction. I needed an M&A firm with the connections to assist me with small, targeted home health investment opportunities that I could grow. Not only did Agenda Health deliver on that need, but they also managed the sale of that large agency at its maximized value. This team is able to excel at both large and small levels in M&A.”
Starting as a small hospice owner with a passion to grow, Agenda Health was instrumental in helping us identify acquisition opportunities and managing the process. Once we grew our company to over 600 patients in 7 states, Agenda found a buyer that allowed us to maximize our value and ensure that our family could retire comfortably. With over 10 years of experience working with their team, I can attest to their commitment to building long term relationships.”
Our family-owned, hospice company was only a small part of our $40 million healthcare enterprise, but it was intertwined with our home health business. Agenda Health worked with us to structure that divestiture and find the right partner. I recommend them to owners looking for a trusted and skilled partner.”
