Lorient Capital-Backed PurposeCare Announces 3 New Acquisitions, Expands Midwest Portfolio
The Lorient Capital-backed PurposeCare has acquired three more Midwestern home-based care agencies. In total, the company has made seven acquisitions over the last year.
Its latest moves include: acquiring the assets of A/Abiding Care-Elder Bridge (AAEB), a private-pay home care agency in Illinois; acquiring equity interests in Michiana Home Care, which provides home health care via two locations in Michigan and Indiana; acquiring the equity interests of Queen City Skilled Care, another home health agency based in Ohio.
“I think strategically these [businesses] are, obviously, in the geographies that we’re in,” PurposeCare CEO Rich Keller told Home Health Care News. “We’re building out a platform to coordinate care between home care and home health care. We’ve got this footprint in the Northeastern corner of Indiana, plus that Southwestern corner of Michigan. We’re continuing to build that Ohio footprint – Cincinnati is a key market for us. And these are high-quality agencies.”

New Day Healthcare Expands Skilled Care Capabilities With Texas Acquisition
Oct. 17, 2023, McKinney, Texas — New Day Healthcare, a rapidly growing home health care provider, has completed its latest acquisition with the closing of Pathfinder Home Health, a skilled care provider in Texas. This acquisition is the eighth completed by New Day Health Care since its inception in 2020.
Founded in 1997, by the Wilcox family, Pathfinder Home Health, has locations in The Woodlands, Beaumont, and Athens, Texas. The organization has consistently delivered high-quality care and exceptional patient satisfaction results.

HealthKeeperz to sell home health business to BAYADA Home Health Care
FAYETTEVILLE, N.C., March 8, 2023 /PRNewswire/ — HealthKeeperz, a community-based health care service provider founded in Pembroke, NC in 1966, has signed an agreement to sell its home health business to BAYADA Home Health Care, a leading not-for-profit home health care provider.
The agreement will transition HealthKeeperz’s Fayetteville and Laurinburg Home Health locations to BAYADA locations and will help BAYADA achieve its mission to care for more clients across the state of North Carolina. The acquisition will allow BAYADA to combine the passionate and talented HealthKeeperz Home Health workforce with BAYADA’s management, expertise, operating platforms, and clinical excellence, to make a large impact on the local community.

Fortis Health Acquires Physiocare Home Health and Hospice
Fortis Health has acquired Indiana-based Physiocare Home Health and Hospice. Financial terms of the transaction were undisclosed.
Grant Avenue Capital, Fortis’ private equity backer, financed the deal, which is the company’s fourth since its founding in 2020. Fortis currently operates in Indiana and Utah. The M&A advisory firm Agenda Health advised Physiocare on the transaction.
“Physiocare is a strong company with an almost 15-year history of serving its community. We were attracted to Physiocare’s impressive home health and hospice services, which aligns with our operating model and is a perfect fit geographically to expand upon our existing services in the Indianapolis market,” Fortis Health CEO Rob Radics told Hospice News in an email. “Their culture and approach to quality patient care are a great fit with what we have established at Fortis Health.”

The Pennant Group Buys Southwestern Palliative Care & Hospice
The Pennant Group Inc. (NASDAQ: PNTG) has inked a deal to acquire Arizona-based Southwestern Palliative Care & Hospice for an undisclosed amount, adding to its existing presence in the state.
Southwestern Palliative Care & Hospice provides home-based end-of-life and serious illness care to communities in Yuma, Arizona. Founded in 2013, the hospice and palliative care provider also offers bereavement services and veteran programs. The M&A advisory firm Agenda Health consulted on the transaction.
The acquisition marks another notch in the home health, hospice and senior living company’s strategic growth plans, according to Pennant CEO Brent Guerisoli, who stated that Pennant will continue to seek growth opportunities in acquiring high quality assets across its business lines.
“We are pleased to continue our growth in the state of Arizona, and complement our agencies in Southern Arizona,” Guerisoli said in a statement. “This acquisition further solidifies our commitment to these communities, and expands our ability to provide life-changing hospice services to residents of Yuma and its surrounding communities.”

The Pennant Group Acquires Guardian Hospice
The Pennant Group Inc. (NASDAQ: PNTG) has acquired Guardian Hospice and Guardian Hospice of Oklahoma for an undisclosed amount.
Guardian Hospice of Oklahoma received Medicare certification in 2015 and has since grown its presence in the state and across Texas. Guardian provides hospice care in 10 counties in southern Oklahoma and across six counties in northern Texas, adding to Pennant’s existing presence in those two states.
Part of what drew Pennant’s interest in Guardian is an opportunity to address unmet needs among rural populations in these regions, according to CEO Brent Guerisoli. The company’s strategic growth plans have included ways to expand the reach of its trifecta of home health, hospice and senior living service lines.
