Lexitas Pharma Services, a portfolio company of QHP Capital, is pleased to announce the closing of its acquisition of Erie Retina Research—one of the nation’s premier retina research centers led by Dr. David Almeida, MD, MBA, PhD—which will operate as a wholly owned subsidiary of Lexitas. As part of the transaction, Lexitas also acquired CASExERIE, LLC, an ophthalmic innovation hub specializing in advanced research, surgical training, and retina-focused product development. CASExERIE will continue to operate within Erie Retina Research. Lexitas is also acquiring the assets of Element Erie: Imaging & Reading Center PLLC, a dedicated center focused on the expert review and interpretation of ophthalmic imaging used in clinical trials.

Superior Health Holdings (“Superior”), a portfolio company of Renovus Capital Partners, today announced the acquisition of Chant Healthcare (“Chant”), a leading provider of home health, homecare, and hospice services headquartered in Southeastern Oklahoma operating through two core entities — Compassion Home Care, which provides home health and homecare services, and Sans Bois Hospice. The transaction marks Superior’s entry into Oklahoma with services in both home health and hospice, along with personal care services across 11 counties.
Chant’s management team, including CFO Derek Lane and COO Stephanie Blair-Bryant, will remain in place to continue to run Chant’s operations.
The acquisition expands Superior’s geographic reach and service mix, establishing an adjacent-state presence in Oklahoma with a platform well-positioned for further expansion toward Oklahoma City and Tulsa. Chant’s large base of dual-eligible Medicaid waiver patients creates a durable referral funnel into higher-acuity home health and hospice services.

Uplift Hospice has acquired Georgia-based Autumn View Hospice for an undisclosed amount.
The transaction brings Uplift Hospice’s average daily census to more than 650 patients, cared for by over 375 employees across Arizona, Nevada, Texas and Georgia. The two organizations connected through an introduction by the M&A advisory firm Agenda Health, which advised the seller through the transaction.
Combining with Uplift will accelerate Autumn View’s growth, according to the seller’s COO Lavell Hewitt.
“Autumn View Hospice has a very solid reputation in the Georgia market. As the hospice landscape continues to change, it was clear that Autumn View needed to align with a strategic partner who understood the culture and growth goals — that partner was Uplift Hospice,” Hewitt told Hospice News in an email. “From Autumn View Hospice’s perspective, we built a very successful community focused growth model in Georgia, and Uplift Hospice saw the potential and wanted to continue that growth.”

DOMA, a provider of intellectual and developmental disabilities (IDD) services, closed June with a deal announcement. The Minneapolis, Minnesota-based company reported that it has acquired Payee Support Services (PSS), an Ohio-based company delivering individualized services for people with IDDs.
The deal strengthens DOMA’s presence in the Midwest and marks its entry into the state of Ohio. Agenda Health, an Austin, Texas-based M&A advisory firm specializing in healthcare, acted as the exclusive sell-side advisor in the transaction.
DOMA provides IDD services with a focus on individualized, community-based care. The company provides an array of community living options, which are Medicaid-waivered services for individuals 18 and older, offering around-the-clock care customized to each person’s needs.
“PSS stood out as an acquisition target due to several compelling factors,” said Elliott. “First and foremost, the organization is exceptionally well-run, with a strong, experienced leadership team that has demonstrated a clear commitment to quality service delivery and operational excellence.”

Creach Family Holdings announced that it has expanded its home health footprint in the Kansas City MSA, with the acquisition of Freudenthal Home-Based Healthcare. The transaction closed in early July 2025 and was facilitated by Agenda Health, an Austin, Texas-based M&A advisory firm specializing in healthcare. Agenda Health acted as the exclusive sell-side advisor to Freudenthal Home-Based Healthcare.
Founded by Joe Freudenthal, Freudenthal Home-Based Healthcare is a provider of in-home health services based in St. Joseph, Missouri. With a strong footprint in Missouri, the company offers a comprehensive continuum of care, ranging from in-home assistance to skilled nursing and hospice services, ensuring seniors receive the support they need at every stage, all within the comfort of their homes.

The home health and hospice company New Day Healthcare has acquired Dunes Hospice in Indiana for an undisclosed amount.
The deal marks New Day’s 16th acquisition to date, as well as its first entry into the Indiana market. The company plans to build density in that state by adding personal care services and skilled home health services to those markets. New Day now serves patients in six states. The M&A firm Agenda Health advised on the transaction.
“The acquisition of Dunes marks a significant growth milestone for New Day as we extend east into Indiana, and it positions New Day for continued state expansion across multiple service lines,” New Day Founder and CEO G. Scott Herman said in a statement. “[Dunes Co-Founders] Wendy [Warner] and Steve [Dobben] have built the top hospice provider in northwest Indiana. New Day’s investment in Dunes brings added resources and strategic expertise to drive growth.”

Active Day announced in April 2025 its acquisition of Illinois-based PremierCare Consulting, marking a strategic expansion in the intellectual and developmental disabilities (IDD) services market. Agenda Health, an Austin, Texas-based M&A advisory firm specializing in healthcare, acted as sell-side advisor to PremierCare.
Based in Naperville, PremierCare is contracted with the Illinois Division of Developmental Disabilities to provide person-centered planning and case management. Its team of Qualified Intellectual Disability Professionals and clinicians delivers in-home and community-based support, helping individuals live more independently through tailored care.
Active Day provides home and community-based services in the United States. Founded in 1988 and headquartered in Trevose, Pennsylvania, the company serves more than 7,500 members, offering healthcare and community services to people with disabilities and older adults. Active Day has pursued a dual strategy of organic growth and targeted acquisitions to expand its reach and community impact.
Active Day’s acquisition of PremierCare Consulting caps a busy April 2025, as the company also acquired South Carolina home care providers AllCaregivers, Inc. and New Generations Home Care of Florence, Inc. earlier in the month. This marks Active Day’s fourth transaction of 2025, following its earlier acquisition of Goldencare, a South Carolina-based adult day and home care provider.
The LevinPro HC team spoke with Matt Donnelly, CEO of Active Day, and Randy Decko, Senior Director at Agenda Health, about what brought these organizations together and the broader trends shaping M&A in this space.
Active Day’s 2025 acquisitions reflect a deliberate strategy to build a national platform integrating IDD and older adult services through in-home, day program and community services. This approach positions Active Day to meet rising demand for comprehensive care models across diverse markets. According to Donnelly, PremierCare stood out because of its strong reputation for person-centered planning and its alignment with Active Day’s mission of enriching community life.

Abound Health is expanding its market presence in North Carolina with the acquisition of Heartspring, a provider of mental health and intellectual and developmental disabilities (IDD) services. Agenda Health, an Austin, Texas-based M&A advisory firm specializing in healthcare, acted as the exclusive sell-side advisor in the transaction. The deal strengthens and expands Abound’s footprint in the IDD market.
Heartspring is a non-profit organization dedicated to providing resources and services for individuals with developmental disabilities. It was founded in 2001 by Richard Williamson. While Abound Health delivers comprehensive services across North Carolina, including day programs, group homes and supportive employment for children and adults with IDDs. Operating for more than 20 years, the company emphasizes person-centered care and community integration to enhance client independence.
Heartspring has carved out a niche in Charlotte with its focus on tailored IDD care. And Abound Health, expanding its behavioral health presence, saw an opportunity to integrate a partner it had long respected. The LevinPro HC team spoke with Alex Veach, Director of Transaction Services at Agenda Health, to discuss how this deal came together.
Heartspring turned to Agenda Health to find a buyer that would keep its focus on tailored IDD care intact. Agenda targeted groups active in IDD, a space drawing steady interest from strategic players like Sevita, which acquired Shorehaven and BrightSpring’s community living business this year, and VIVA Pediatric Healthcare, which bought two Texas pediatric care centers in March 2025.
“Agenda facilitated a competitive market process for Heartspring that produced multiple offers,” Veach said. “Heartspring ownership decided that the offer received from Abound Health best aligned with their exit goals.”
