Home Health
We help both medical and non-medical home health practices assess their market value and position their organizations for sale while preserving their commitment to quality patient care.
As faith-driven advisors, we offer transparent M&A guidance to owners like you, honoring both your business goals and your calling to deliver unparalleled in-home care services.
Mission-First Home Health M&A
As a home health provider, you’ve built more than just a business, you’ve created a vital lifeline of care that supports patients in their most familiar environment. At Agenda Health, we understand that selling your home health organization isn’t just a transaction; it’s about ensuring your standards of quality care continue. Our team has shepherded many home health owners through this most important transition, helping them find culturally fit buyers in home-based care and negotiate a fair sale.
Navigating Medical Home Care’s Evolution
Having worked alongside many Medicare home health agencies over the years, we’ve gained a nuanced understanding of what drives success and what creates challenges in this evolving space. As referral patterns shift and Medicare Advantage enrollment surpasses traditional fee-for-service, agencies are adapting to new expectations around outcomes, documentation, and reimbursement.
Operators with strong clinical protocols, efficient intake processes, and diversified referral sources continue to attract interest from buyers, particularly those positioned to scale within the changing landscape. Our experience in this space allows us to help founders navigate these shifts and make informed decisions about timing, structure, and strategic direction.
Leveraging Growth in Non-Medical Home Care
With a strong track record advising non-medical home care businesses, we bring deep insight into the operational and financial levers that matter most, whether an agency is private pay, Medicaid-based, or a hybrid of both. As more seniors choose to age in place, demand for non-medical home care continues to rise, supported by both demographic trends and shifting consumer preferences.
Investors remain active in this space, particularly around scalable, tech-enabled operators with strong retention and caregiver networks. We’ve helped owners across the spectrum, from single-location agencies to multi-state platforms, navigate successful transactions and growth strategies, and we’re well-equipped to guide you through what’s next for you as well.
“Our family-owned, hospice company was only a small part of our $40 million healthcare enterprise, but it was intertwined with our home health business. Agenda Health worked with us to structure that divestiture and find the right partner. I recommend them to owners looking for a trusted and skilled partner.” – Troy Ayers CFO of Healthback Home Health

2026
Superior Health Holdings (“Superior”), a portfolio company of Renovus Capital Partners, today announced the acquisition of Chant Healthcare (“Chant”), a leading provider of home health, homecare, and hospice services headquartered in Southeastern Oklahoma operating through two core entities — Compassion Home Care, which provides home health and homecare services, and Sans Bois Hospice. The transaction marks Superior’s entry into Oklahoma with services in both home health and hospice, along with personal care services across 11 counties.
Chant’s management team, including CFO Derek Lane and COO Stephanie Blair-Bryant, will remain in place to continue to run Chant’s operations.
The acquisition expands Superior’s geographic reach and service mix, establishing an adjacent-state presence in Oklahoma with a platform well-positioned for further expansion toward Oklahoma City and Tulsa. Chant’s large base of dual-eligible Medicaid waiver patients creates a durable referral funnel into higher-acuity home health and hospice services.
“This acquisition is a natural next step for Superior,” said David Martin, CEO of Superior Health Holdings. “Chant gives us an immediate presence in Oklahoma — in home health, homecare, and hospice — and a management team that knows these communities deeply. The combination of our operating capabilities and Chant’s local relationships positions us to grow meaningfully and serve more patients across the region.”

Insights from Those We’ve Served
Hear from those who have gone before you and learn how our unique approach has helped healthcare organizations like yours achieve their transition goals and sell their business.
Having spoken with several agencies before making our decision, the transparency of Agenda stood out. That made it real simple: if we were going to do business, we wanted to do business with a firm of that integrity. They were present at every meeting, and the prep and debrief conversations meant we always knew what to expect. As we got closer to the end of the deal, it was new terrain for me and I didn’t know how to navigate it, but Agenda did a great job helping us work through those difficult talking points. In the end, they came through with everything promised on the front end, and that exceeded expectations.
Initially, the process felt overwhelming since it was my first time selling a business, but Agenda Health helped ease my concerns and answered all my questions. Agenda filled in the gaps where I lacked understanding … in the end, Agenda made a very complicated process feel simple.”
The Agenda Team was patient and went at the pace that we needed to go, which was important for us. They also provided education on the various dynamics of the process, which we greatly appreciated. We were introduced to a variety of buyers, which opened our eyes to various possibilities and gave us a good idea of what we were looking for in an acquirer. We would highly recommend Agenda for the value they bring.”
My overall experience working with Agenda was very positive. The team was professional, supportive, and ultimately delivered a great outcome. The valuation they provided was very insightful and valuable in understanding my company’s position, and I really appreciated the constant communication throughout the process, which kept me informed at every stage. Overall, the guidance, insights, and consistent support were invaluable in reaching a successful outcome, and I’m pleased with the experience.”
Agenda really understands ABA – they understood the differences in my practice and others and had great knowledge of the ABA-specific M&A market. They were able to clearly express our mission and program to potential buyers and understood what made my business different. Everyone was aligned on my goals and worked hard to accomplish them.”
Working with Agenda Health was a first-class experience. As first-time sellers, their clear communication and steady support helped calm our nerves throughout the process. We hadn’t planned to sell until Agenda provided a valuation, but we never felt pressured in this process. After Agenda presented our valuation information and the pros and cons of selling, we felt equipped to make an informed decision and committed to partnering with Agenda to sell our business.”
Agenda Health’s deep understanding of the ABA space and knowledge of the market made all the difference in my transaction. They knew exactly how to position our business and connected us with the right partner quickly. Agenda’s expertise gave us confidence every step of the way, and the outcome exceeded my expectations.”
Agenda gave us peace of mind and ease in selling our business. It’s such a big decision and undertaking, and Agenda takes something that’s probably one of the biggest decisions that a company owner is going to make and makes it easy and smooth. I would highly recommend it to anybody that is in our same kind of situation.”

I needed an M&A firm with the connections to assist me with small, targeted home health investment opportunities that I could grow. Not only did Agenda Health deliver on that need, but they also managed the sale of that large agency at its maximized value. This team is able to excel at both large and small levels in M&A.”
Starting as a small hospice owner with a passion to grow, Agenda Health was instrumental in helping us identify acquisition opportunities and managing the process. Once we grew our company to over 600 patients in 7 states, Agenda found a buyer that allowed us to maximize our value and ensure that our family could retire comfortably. With over 10 years of experience working with their team, I can attest to their commitment to building long term relationships.”
